For emergencies call 911, or going to the nearest emergency room. There is a crisis assessment center at Poudre Valley Hospital at 970-495-8090 which can also be helpful.

Insurance:

Does Dr. Daine accept insurance?

Currently, Dr. Daine is considered an out-of-network provider for all insurance companies. Prior to scheduling an appointment with Dr. Daine please contact your insurance company to see what they reimburse for out-of-network providers.

If you wish to utilize insurance benefits, we will provide you with a “superbill” so that you may submit a request for reimbursement.  The type of reimbursement that you are eligible for will depend on your insurance, network constraints and the nature of your deductible for services.

The providers within our practice are out of network on plans.  The reasons for this are complex; basically, we can practice without restrictions on the length of sessions or the number of visits we can see you.  Dr. Daine is not restricted by very short visits and we can provide the highest standard of care this way.

Does Dr. Daine accept Medicare or Medicaid?

Dr. Daine is opted out of Medicare and is not a Medicaid provider Dr. Daine welcomes you to schedule a visit but you will be responsible for the fees as above and will not be reimbursed by Medicaid or Medicare.

Overdue Accounts / Other Fees :

Accounts that are past due may be referred to a collection agency as a last resort.  Any fees, losses or charges including collection fees or legal fees, up to 50% of the amount owed, made while securing past due balances will be added to any overdue accounts.

Checks that are returned as unpaid by your bank will result in a $30.00 charge.

In regards to requests for responding to a subpoena and requests for records or summary of your records, there is a fee for each that will be added to your account.




Appointments are by arrangement. 
Some evening and Saturday appointments may be available.

Payment

Cash, checks  and credit cards accepted.

For your convenience, I accept credit card payments  in my office and online through PAYPAL. You do not need a Paypal Account to use this service.  If you would like to make a payment in advance of your appointment, you may do so by choosing the amount and clicking on the Add to Cart button.  

If you don't see the exact amount you want, you can select more than one dollar amount in the various increments to get to the total you would like credited to your account.  For example, if you want to charge $105.00, you can chose $100.00 and then return to Keep Shopping to add $5.00 to your cart.  When you have reached your total amount you can proceed to Check Out.
